Please complete the on-line Roommate Selection Questionnaire by May 22nd, 2009.
You must have Adobe Reader 9.0 or higher in order to complete the on-line form!!
It is not to be downloaded as a pdf and placed on your desktop or e-mailed - it is to be completed and then submitted on line by clicking the submit button which should appear in the top right-hand corner of the purple bar.
If you do not have Adobe Reader 9.0 or higher, please download adobe reader before you begin. You also need to have Adobe Flash player installed.
This file should be opened in a web-browser and works best with the following web-browsers: Internet Explorer 7.0, Safari v 3.2.1, or FireFox v 1.5.0.6.
Please read the following steps thoroughly before beginning.
- Click the download button
- do not save as a pdf to your desktop
- Complete the form and select the Submit button (top right hand corner)
- You will have to fill in your email address and name. You must use your Centre email address.
- Click the send button
MACINTOSH USERS:
If you are on a Macintosh computer and you get a dialog box that prompts you to open the file, please choose Adobe Reader. If you choose Preview, you will not be able to fill out the form or complete it.
After you install Reader, open Reader and see if you get the following message “reader is not set to open pdf files, do you want reader to open pdf files instead of preview”, click Yes. Now you should be able to complete the form.
NOTE: If pdf files are opening in preview mode or saving on your desktop in preview mode, you need to change the file type so that pdf files open in Adobe Reader.
